Renfrew County Youth Wellness Hub Care Coordinator/Navigator
Permanent Full Time The Renfrew Victoria Hospital (RVH) is a 55-bed, fully accredited facility located just 45 minutes west of Ottawa, serving a catchment area of over 60,000. Part of the Renfrew and Area Health Village, RVH provides medical, surgical and complex continuing care as well as extensive ambulatory care with over 30,000 emergency and clinic visits per year. Job Summary The Care Coordinator will, in collaboration with youth and their families, assess care needs, determine eligibility for services, and develop individual care plans to help youth receive the services and supports they need. Responsibilities ● Create opportunities within the hub for youth to connect with YWHO partners and build awareness of available services and supports ● Provide outreach and facilitate continued engagement of youth at YWHO ● Provide support and follow up to youth ● Refer any youth in need of crisis, mental health, or other core service supports to clinical staff ● Provide referrals/make linkages to culturally appropriate services/supports ● Assist youth in completing any necessary forms or requirements for services ● Document and maintain electronic youth client records (intake and assessment forms, progress notes, correspondence, client files, etc.) ● Work with other staff to help identify when youth may require supports- either in terms of mental health or substance use ● Build and maintain relationships with network partners and other relevant services ● Contribute to creating and maintaining a safe, welcoming, non-judgmental, and non-stigmatizing hub atmosphere ● Work from a harm reduction, collaborative/team-based and youth-centered approach ● Other duties may be required
Qualifications Qualifications/Skills/Experience ● 2-3-year diploma from a college in mental health and addiction attachment to a college is a requirement. ● 2+ years of recent experience in community health, social services, or related field ● Excellent communication, interpersonal, assessment and decision-making skills ● Fluency in French is an asset ● Can work variable hours that might include evenings, weekends and weekdays ● Valid First Aid & CPR certificate or willingness to obtain. ● Must hold a valid driver’s license and be able to travel to multiple Renfrew County YWHO locations when required ● A satisfactory and current Vulnerable Sector Screening ● The Renfrew County Youth Wellness Hub Ontario has a mandate requiring all employees to be fully vaccinated against COVID-19.
