Your medical record is confidential. It contains information from your visits to Renfrew Victoria Hospital, including treatments received, examination and test results, and Emergency, Outpatient and Inpatient visit details.
- You must provide written consent for RVH to share or disclose any information from your medical record. You can find the authorization form here.
- You can access your own medical and personal health information anytime, anywhere through the secure online patient portal MyChart.
Accessing your personal medical information
As a patient, you may access your personal health information and medical record by submitting a written request. You can find the form here.
You may also request that information contained in your medical record be corrected.
