Palliative Care Program

Palliative care offers individualized care and promotes personal dignity and improves the quality of life of patients and their families facing a life threatening illness.

The Renfrew Victoria Hospital Multidisciplinary Palliative Care team consists of doctors, nurses, social worker, pastoral care, physiotherapist, respiratory therapist, recreational therapist, recreational therapist, dietitian, speech language pathologist, CCAC case manager and volunteers.

The Palliative Care Team will work collaboratively with patients, their families and the primary care team to enhance quality of life for patients by providing physical, spiritual, and psychosocial support to the patient and families.

The Team will ensure comfort, compassion and dignity for complex symptom management at the end of life respecting the patient's choice and place of care.

When can Palliative Care be requested?
Palliative care services can be requested when the patient, family or health care professional realizes that the patient is terminally ill.

Who refers patients to Palliative Care?
Referral to palliative care is made by your physician. Please feel free to discuss palliative care with your physician.

What services are available?
The following services can be provided by health care professionals, and trained, dedicated volunteers.

  • patient and family consultation and support
  • pain and symptom control
  • care at home or in hospital
  • scheduled relief for the family or other care givers
  • bereavement follow-up
